measurement of patient progress and outcomes and displays such information to
patients and their health care provider(s). The purpose of this Privacy Policy
is to set out the manner in which the Company collects, uses, discloses and
otherwise manages personal information. This Privacy Policy applies to the
privacy practices at communityalt.portal.gs (“Website”) and offline collection
